Advancing Practice Through Evidence: Upcoming AAGL Guidelines in JMIG

The AAGL Practice Guidelines Committee is pleased to share two forthcoming publications in JMIG that reflect the Society’s continued commitment to advancing evidence-based care in gynecologic surgery.
The first, “Evidence-Based Guidelines for Instrument and Handle Selection for Ergonomic Optimization in Laparoscopic Surgery,” addresses a critical and increasingly recognized dimension of minimally invasive surgery – surgeon ergonomics. As laparoscopy has improved patient outcomes, the physical demands placed on surgeons have grown in parallel. Musculoskeletal strain, fatigue, and cumulative injury are no longer peripheral concerns—they directly impact surgeon performance, career longevity, and ultimately patient care. This guideline synthesizes the available evidence and underscores the importance of intentional integration of ergonomic principles into laparoscopic instrument design and surgical practice.
The second, “Visually-Directed Hysteroscopic Biopsy in the Evaluation of Abnormal Uterine Bleeding and Postmenopausal Bleeding: A Joint Society Practice Guideline,” represents a collaborative effort among AAGL, the Global Community of Hysteroscopy, and the European Society for Gynecological Endoscopy (ESGE). This work reflects a global consensus on the evolving diagnostic approach to abnormal uterine bleeding. Drawing on a rigorous review process, the guideline highlights the limitations of blind endometrial sampling and supports the integration of hysteroscopy—with visually-directed biopsy—into current diagnostic algorithms. By enabling targeted evaluation of intrauterine pathology, hysteroscopy enhances diagnostic accuracy and aligns practice with the principles of precision medicine.
The completed guidelines represent the culmination of more than 2.5 years of sustained effort by a large group of contributors across the country and internationally—work that is only possible through the dedication, intellectual rigor, and steadfast commitment of experts who have invested their time to advance the field. As we continue to move forward, AAGL remains deeply committed to translating evidence into clinical practice in ways that elevate care, support innovation, and uphold the highest standards in women’s health.
